Scope of Work The Architect/Engineer (A/E) firm shall furnish professional services for 100% design Correct Cantilever Near MRI Building at VA Northern Indiana Health Care System, Fort Wayne Campus, 2121 Lake Avenue, Fort Wayne, IN 46805-5100. The design will include Schematic Design, Design Development, Construction/Bid Documents per the A/E Design Submission Instructions Document, Construction Administration Services, Technical Specifications and Cost Estimates. This project will investigate the structural damage on the cantilever to the entrance of Building 1 near the MRI Building and provide a design to correct the deficiencies noted. Review existing electrical, water, HVAC, and Stem piping that currently flow under the existing slab. Repair of existing cracks due to settlement of foundation and correction of exposed metal rebar.
