Request for Proposal (RFP) 36C24225R0047 The Samuel S. Stratton VA Medical Center (Albany VA Medical Center) of Veterans Integrated Service Network (VISN) 2 of New York/New Jersey has a need for Community Based Outpatient Clinic (CBOC) services. The CBOC services consist of primary care, telehealth and podiatry services to Veteran patients in an office or clinic environment. The CBOC must be located in Kingston, NY. The contract will cover a base year and four (4) one-year option periods to be exercised at the Government's discretion. No hardcopies will be sent. Questions and proposals must be received by the dates indicated on Page 1 of the solicitation. If warranted, responses to inquiries will be incorporated into a written amendment.
